(1) In the case of choosing coverage in the 5 million baht plan, the limit is per one-time treatment.

(2) In the case of choosing the coverage for cancer and chronic kidney failure, the 5 million baht plan, in the amount per year and will cover medical service fees for the treatment of cancer and/or chronic kidney disease for treatment methods as determined by the Company.

• Additional coverage: Maternity Plus and Good Health Plus. Coverage areas Only Thailand
• An additional health insurance contract, D Health Plus type, must be purchased attached to the policy that is currently in effect.
• The coverage of the additional contract must not exceed the insurance period of the life insurance policy to which this additional contract is attached.
• The record endorsing maternity coverage Plus (Maternity Plus) and the record endorsing Good Health Plus (Well-Being Plus) must be purchased attached to the additional health insurance contract, Elite Health Plus and D Health Plus only. The coverage of the endorsement record must not exceed the insured period of the additional contract to which the endorsement record is attached.
• Room, board, and daily nursing service fees (not exceeding the cost of a standard single room) for a maximum of 180 days per one hospital stay.
• Intensive care unit (I.C.U.) cost up to a maximum of 60 days per stay.
• Medically necessary take-home medicine for no more than 7 days, but not more than 20,000 baht per one hospital stay.
• The waiting period for common illnesses is 30 days.
• Insurance underwriting is based on the company's criteria.
• Insurance premiums can be used for tax deduction rights, provided the criteria are as specified by the Revenue Department.
